چکیده

A 72-year-old woman presented with intermittent diplopia. She had a history of vascular disease, but no history of stroke or psychiatric disturbance. On examination, she developed convergence spasm with associated miosis following fixation on near targets and during horizontal smooth pursuit (video, figure 1). The spasms were terminated with a blink. She had no ptosis, her eye movements were normal in range, and her pupils were equal in size with normal reactions to light. Neurologic examination was otherwise unremarkable. Five years prior to her current presentation, a left-sided terminal carotid artery aneurysm had been incidentally detected. The aneurysm gradually expanded to a diameter of 2.4 cm and compressed the midbrain (figure 2). The intermittent diplopia had developed in the months following endovascular coiling of the aneurysm. Convergence spasm is characterized by the inappropriate appearance of the near triad, which consists of convergence, miosis, and accommodation.1 Most patients are young and the cause is psychiatric; a focal lesion is rarely found.1 Since the neural substrate for the near triad is located in the midbrain, convergence spasm could occur Address correspondence and reprint requests to Dr. G. Hemifacial spasm and recurrent stroke due to vertebrobasilar dolichoectasia coexisting with saccular aneurysm of the basilar artery: a case report.

We describe a patient presented with sequential events of hemifacial spasm, cerebral infarction and fatal subarachnoid hemorrhage. All of them are seemingly separate entities. Radiological examination revealed that the cause was vertebrobasilar dolichoectasia (VBD) coexisting with a giant saccular aneurysm of basilar artery. VBD is a dilatation and elongation of vertebral artery (VA) and basila...

Convergence spasm due to aquaporin-positive neuromyelitis optica spectrum disorder

A female 27 presented with nausea and diplopia for 1 week. On examination she had normal vertical gaze but would develop convergence with miosis whenever she made horizontal saccades. Pupils were 6 mm and unreactive to light. MRI showed extensive hyperintensity in the dorsal midbrain and thalamus. Spinal MRI and CSF were both normal. Serum aquaporin-4-antibody was positive. She was treated with...

Cerebral vasospasm with infarction.

Cerebral Vasospasm With Infarction • A case of intense unilateral cerebral vasospasm with appropriate neurological deficit following rupture of a posterior communicating aneurysm was followed with serial angiography. Associated with the initially intense spasm was subsequent regional hyperperfusion with early venous filling which disappeared as signs of focal cerebral atrophy ensued. These angi...

Acute syndromes include Parinaud’s syndrome (vertic...
